Risky disarray at VA hospital

Shortly after an Office of Inspector General report found unsanitary conditions and inadequate equipment at a D.C. medical center, the director of the facility was promptly fired.

In a matter of three hours Wednesday, the VA appointed one acting director from inside the hospital, then another from outside, Lawrence Connell, a retired Army colonel who was a senior VA policy adviser.

The OIG conducted its investigation of Washington DC VA Medical Center at the end of March and returned for a second visit during the first week of April, according to its report (PDF). While looking into this problem, investigators found that out of 25 sterile storage areas, 18 were dirty.

The VA said effective immediately, the director of the medical center, Brian Hawkins, has been relieved from his position.

"The OIG's work is continuing and will include an assessment of whether patient harm has resulted from any of these inventory practices in its final report on the Medical Center", he wrote.

The medical center recorded 194 patient safety reports since 2014 related to unavailable equipment or supplies, the report found. The VA placed the head of the medical center on leave Wednesday afternoon and brought in a senior agency official and additional staff.

"When I became aware that veterans were at risk, that's when we took immediate action", Shulkin said in a Thursday news conference. In one case, the hospital ran out of kidney dialysis equipment and had to ask for the tubes from a private sector hospital.

The launch came after former VA Secretary Bob McDonald reluctantly released performance ratings a year ago for each of the agency's hospitals.

Missal said that these inventory systems and procedures should be standardized across the Veterans Health Administration.

The inspector general rarely issues such preliminary findings.

The VA's Secretary David Shulkin said during his confirmation hearing that transparency is critical to fixing problems with wait times at hospitals.

In comparison, a VA facility in Murrieta has a 37-day waiting period for a patient to be seen - the longest wait time in California.

The website is created to increase transparency and care quality of the VA, which has been rocked by controversy for the way it handles wait times and patient records.

Dawn Jirak, a deputy director at Veterans of Foreign Wars, said her organization is anxious that "this is just yet another website that VA has come up with" but said she looked forward to improvements the VA says it will make to the site. The 2.2 million members of The American Legion demand that President Trump and Secretary Shulkin hold those officials responsible for the gross mismanagement of this facility and accountable for their actions.
